VALID PERSONAL CHECKING OR SAVINGS ACCOUNT

Applicant must have a valid personal checking or savings account in their name that has been open for a minimum of 6 months.  Applicant may be asked to submit the following if needed:

1. First page of a bank statement from 6 months ago from the same account. This must have at least last four digits of account number (only needed if verification of bank account information is required).
2. First page of transactions from last 24 hours. This must have a positive balance and at least last four digits of account number (only needed if verification of bank account information is required).
3. A bank statement including full account number and customers name if not satisfied by “1” or “2” (only needed if verification of bank account information is required).

VALID GOVERNMENT ISSUED ID

This can be a Drivers License, ID Card, Passport, Matricula Card, Military ID, or other government ID

VALID SOCIAL SECURITY #

Document to verify social security number (only needed if verification of SSN is required) such as:

• Social Security Card
